I came up with a bunch for the White Sox through newspaper archives, most with specific dates, all with a year.
Majors:
Bill Simas (Dec. 22, 2000)
Jason Bere (Sept. 13, 1996)
Minors:
John Snyder (July 24, 1996)
Lorenzo Barcelo (Sept. 1998)
Rocky Biddle (March 2, 1999)
Kris Honel (2005, after the season)
Mitch Wylie (1999)
Jason Stumm (2000)
You have Randy Williams’ surgery attributed to the White Sox, but he was in the Cubs organization at the time.
Non-Sox I found while looking up White Sox:
STL — Scott Radinsky (June 10, 2000)
ATL — Mike Bielecki (Aug. 2, 1992)
Disregard Stumm — I see you already got him.
I believe Tony Peña had TJ surgery as a White Sox.
Nick Hagadone June 2008 Red Sox minor leaguer
JM Gold 2000 Brewers minor leaguer
Jeremy Bonderman April 2012 Tigers
Mitch Conner June 2012 Brewers minor leaguer
Mike Jones June 2007 Brewers minor leaguer
Jose Valdivia 2010 Mariners minor leaguer
Erik Cordier Dec 2006 Royals minor leaguer
Erubiel Durazo July 2005 A’s DH-1B
Casey Crosby Nov 2007 Tigers minor leaguer
Hector Gomez June 2008 Rockies minor league SS
Brad Hawpe Aug 2011 Padres OF-1B
Tsuyoshi Wada May 2012 Orioles
Jose Casilla July 2011 Giants minor leaguer
Drew Naylor May 2011 Phillies minor leaguer
Cam Bedrosian May 2011 Angels minor leaguer
Cesar Carillo 5/15/07 Padres minor leaguer
Eddie Bonine 2011 Phillies minor leaguer
Jarrod Parker 10/28/09 Diamondbacks minor leaguer
Eric Milton June 2007 Reds
More than once:

Darren Dreifort- also in 1995 Dodgers
Steve Ontiveros- 7/20/89 Phillies and 6/13/96 A’s

Thanks for all of the new updates, especially mls for that extensive list. All above this point in the comments have been added to the confirmed list.

Erick Threets (row 173 on the TJ list, not 172) has to be incorrect. On 8/28/2012 He he was assigned to the Dodgers’ AAA affiliate Albuquerque, not SF. Not only is he currently pitching in the DWL for Estrellas de Oriente, but he also pitched for Albuquerque on 8/27, 8/31, 9/2, 9/5, 9/7 and 9/9 of 2012 which conflicts with the date of the TJ surgery you have listed.

First and foremost, this list is amazing. Thanks for all the hard work.
I noticed that you have Carlos Almanzar listed as having TJS in September 2011 with the Rangers, but everywhere else seems to indicate he had the procedure back in 2005 when he was still with the Reds.

Tiny correction: Juan Pena made two spectacular starts with the Red Sox in 1999, then went on the DL with an elbow injury. He tried rehabbing, was shut down, and had TJ in the offseason. So the “Major League” column should actually be Y. If someone were looking for the shortest MLB career ended by an injury requiring TJ, he’s certainly a contender.

Paul Molitor was the first position player to have TJ surgery, in May 1984; he’s not listed but should be. http://www.twincities.com/twins/ci_25256030/twinsights-paul-molitor-can-relate-miguel-sanos-tommy
He’s also the only player in the Hall of Fame who had it. John Smoltz will likely be the 1st TJ pitcher in HOF
Also, Luis Gonzalez had TJ on 8/2/2004 http://arizona.diamondbacks.mlb.com/ari/news/ari_press_release.jsp?ymd=20040802&content_id=817693&vkey=pr_ari&fext=.jsp
Luis Gonzalez had TJ 8/2/2004
Thanks for the notes on Paul Molitor and Luis Gonzalez. I’d added Molitor a few weeks back, but have now added Gonzalez as well.

Dear Sir,
Tom Candiotti, then minor leaguer form Milwaukee Brewers, had Tommy John surgery in 1981. He went on to pitch for Cleveland Indians and Los Angeles Dodgers. Dr. Frank Jobe did his surgery. He is not on your list. Reference-article on Tommy John surgery in Chicago Tribune February 24, 2000.
Tampa Bay Rays prospect Grayson Garvin should also be on the Tommy John list. He underwent surgery in 2012 and there appears to be a multitude of places where it says that he did.
Thanks for the tips on Candiotti and Garvin. I’ve long had Candiotti on my “Unconfirmed” tab as I hadn’t seen a date for his surgery. Thanks to both of you, I found a few others while confirming these two surgeries as well that I added to the list. Older surgeries and MiLB surgeries are of course the hardest to find/confirm.

Too bad there isn’t leftie vs rightie info. Would be great to get that info. Any idea how that could be sourced?

Thanks Jay, added Teagarden.
This is a really interesting list. From an analytic standpoint, it would be interesting to know the age they had their surgery, where they grew up, and if they played any other sports.

A couple notes:
• As best I can tell, Ben Davis underwent surgery on 6/28/2005 (listed as 1/1/2005) http://tinyurl.com/qgrptsr
• J.R. House’s surgery couldn’t have been on 9/1/2003, as he made his MLB debut later that month. Research via Baseball America shows that it happened sometime in September 2002 http://tinyurl.com/oojb8m6

A correction: David Palmer’s TJS was actually September 1982: http://tinyurl.com/pvkw38r He did have elbow surgery in November 1980 but Baseball Prospectus appears to be incorrect in listing that one as the TJS.
Also Brent Strom’s surgery was in 1978, not ’77: http://tinyurl.com/lvannrb He says he had already been released by the Padres when he had it.
We can also add minor leaguer Dwayne Henry in July 1982: http://tinyurl.com/mlhw8ew
Last few entries, I promise!
Japanese player Masaharu Mitsui in 1979: http://www.baseball-reference.com/bullpen/Masaharu_Mitsui

Larry Christenson? Baseball Prospectus lists a 6/15/83 procedure as TJS but I did not find any contemporary accounts detailed enough to confirm that. http://tinyurl.com/npe3rz4
